The problem as I see it is that the Target Concepts of Pain Revolution are not supported by any published scientific research.
They appear to be speculations/assumptions presented to those experiencing pain as if they were the truth or established knowledge.
Does this really matter?
I believe the answer is a resounding YES.
This is because one of the more powerful therapeutic tools available to the clinician, irrespective of their background discipline, is to present a clear and honest explanation that is as close to accurate as possible in the current state of knowledge.
Of course it matters, but what can patients really do in the course of their search for relief which is their number one priority?
As stated in my blog https://pudendalnerve.com.au/2022/02/02/and-the-elephant-in-the-room-is/ this ‘inflammation’ explanation and some of the more holistic explanations have been the most helpful for me over my 15-year experience. You words stay with me many times but so do some of the explanations in the Protectometer publication.
I will never forget you telling me that I knew more than any researcher present at the first pain conference I was invited to present at. I also won’t forget my Chinese Medical Practitioner explaining my injury is first and foremost a shock to the nervous system. These insights and feedback have been the most calming, and most affecting for me. When I read the research, it just makes no sense no matter how hard I try to get my head around it.
Perception or sensation is a great way to present the battle between Quintner / Pain Revolution. But it still leaves a lot of us in the same painful situation.

Soula, “trust” is a foundational element in any successful clinician-patient engagement.
When one of the pair passes on misinformation to the other as if it were true, trust cannot be established between them.
This is why we challenged the “Target Concepts” of Pain Revolution.
By failing to respond, the Pain Revolution theorists have betrayed the trust they so earnestly desire from patients and health care professionals alike.

After almost 2 years, there has not been a response from these theorists.
I have listed your publication on the site John. It is important for followers to read the research… if they can! As a patient, it’s very hard to understand the research and know where to go or who to listen to. I call myself well-read and there are days where I just can’t stand to read one more theory, I just want the help.
Personally, I relate to parts of both of your theories. At times it changes too. It really is hard from where patients stand.
